Your guest intake form lives at yourwebsite.com/guests/intake/ — that's the link to send guests so they can fill out their own profile before their episode goes live.
You can also find it in your dashboard: go to Pages → Guest Intake Form, where you can view the live form and copy its address. This video shows where to find and copy the link:
