Yes. The Guest Intake Form collects a standard set of details from your guests (name, bio, photo, and links), and you can extend it in two ways: add your own custom questions, or link out to a separate questionnaire.

How to add custom questions
You can add up to three custom questions to the form:
- In your dashboard, go to Pages → Guest Intake Form and click Edit Page.
- In the Custom Questions section, enter your question in one of the Question fields.
- Choose the answer type — Text, Multi-line Text, Email, or URL — and whether the question is required.
- Click Save.
Your guests' answers are saved with their profile in your dashboard and included when you export your guest list.
Link to a longer questionnaire
Need more than three questions? In the same editor, add a link in the Additional Questionnaire section. After submitting the intake form, guests are pointed to your external form (Google Forms, Typeform, Tally, and similar tools all work), so you can collect anything specific to your show.
The standard fields themselves can't be removed or reordered.
