When Breathing Stops In Sleep
SPEAKER_00 0:00
People literally stop breathing multiple times during the night. When you are not able to breathe, the first thing the body has, found a solution is to open your mouth because the nose is not doing its job. It's a battery effect in the mouth right next to the brain. An infection in the bone is gonna sit there for decades, untreated, undiagnosed. There can be bacteria, fungus, mold, parasites, and that's what I clean when the infections are sitting there.
SPEAKER_01 0:32
Without oxygen, it brings disease and then death, early death.
Live From Cannes With Dr Sachin
SPEAKER_01 0:38
Live from Cannes, France, with another amazing topic, one of my favorites. How does your airway affect your brain, your health? Could it even cause autoimmune? Well, I'm here with an expert, and we're gonna talk about a lot of cool dentistry because of your background. All right, Dr. Sachin, let me say it correctly. It's like uh currande, currande. He gave me a good analogy. I should have used it. Okay.
SPEAKER_00 1:02
Yeah, it's like Grande Carrande.
SPEAKER_01 1:04
Yeah, we'll go with Dr. Sachin. I said with I'm a dyslexic. When I read it, it said Sachin. S-A-C- Uh C-H-I-N. See how many.
SPEAKER_00 1:11
You're saying it perfectly right, Sachin. Thank you so much.
SPEAKER_01 1:14
Okay, so you grew up in Bombay, and I want to start there because that really kind of pulls full circle of a lot of your take on what we're going to talk about. Absolutely. How this can cause disease and how it can affect not just your physical body, which people don't even understand that, but it can affect your whole body brain spirit, everything. So talk about your background. Grew up in Bombay.
SPEAKER_00 1:38
So I grew up in uh Mumbai, Bombay.
SPEAKER_01 1:41
Bombay, what do I say? Is that you're saying right. I mean, I'm used to saying Bombay. Okay, yeah.
SPEAKER_00 1:46
It became but there's a story to that too, which is Mumbai Devi was a goddess or there, a local deity, and the name comes from there. So the the locals called it Mumbai, and then it was changed to Bombay. For me, it's always Bombay. Okay. So going back to the airway, there has been a passionate journey that happened in my life when uh I became a dentist, and I started realizing that the path to understanding was further
Snoring As A Personal Wake Up
SPEAKER_00 2:14
education, further education, but at the same time, it was necessary to understand that people who are coming and sitting in your chair are getting a certain amount of healing, but they're not healing to their fullest. So where is the gap? That pushed me to find out my own personal journey when my younger daughter calls me one night and said, Daddy, you're snoring. She was with me and my wife had taken the older one to her post in Jersey that night in Virginia. So I had made an appliance out of curiosity to understand sleep, TMD, and I used that appliance that night, and then the next day morning, my daughter didn't complain, and I had to push her to ask me that question. Did I snore? And I casually asked her or breakfast, said, Sweetheart, did I snore last night? No, you didn't, Daddy. And my personal observation, I've been a very avid observer of my breath, born in a yogic tradition, seeing my grandpa meditate for eight to ten hours. She was a school teacher, but all that journey of spiritual uh yogic and chronic movement of the breath was just an actual thing.
SPEAKER_01 3:26
I mean, you grew up understanding breath and how to breathe, etc. And here you are with a breathing issue, unknowing to yourself, your daughter tells you.
SPEAKER_00 3:33
Absolutely. So that was a wake-up call. And as I said, during that time, I felt like I came out of a two-hour meditation with that sleep that one night, and it just changed my whole paradigm about how. Just from a simple device that I had fabricated as an experiment on myself, and I used it that night, that really made me think there is more to know here. And the journey deepens as I was diagnosed with rheumatoid arthritis, and they wanted to pump how old are you? I I'm 46 right now. Okay, but then with the diagnosis. Almost like 15 years ago. And they wanted to take those medications that will shut down my whole immune system, biologic system. Not being a physician, you knew this was not. That's not the way. Took Cortisan shots once while once once a time, and as I said, they are harming my best friend, which is my immune. This is not the way I was trained and understood. We think food is medicine, movement is energy, and rest is refueling. Those are the things I was born up with, brought up with. With that passion, I said I'm gonna heal myself. My journey went deeper and deeper, understanding breath work, the connection of the jaw, because every time you're not taking that right breath, there is a trail that is left on your teeth, and that every scar, that every scratch a tooth does on the opposite tooth is a story that I can see. Okay, I don't understand that. Explain that further.
Mouth Breathing, Tongue Collapse, Apnea
SPEAKER_00 5:05
So great. So when you are not able to breathe, the first thing the body has to found a solution is to open your mouth. Because the nose is not doing its job. So now your mouth breathed. So now your mouth breathed.
SPEAKER_01 5:18
And by the way, so that's why people are taping their mouth. But I always I'm a critic of that because I say, well, wait a minute, hold on. Your brain's trying to get oxygen to live. That's why it's opening its mouth. But is that the cause to tape it?
SPEAKER_00 5:31
That makes you a wise man because you cannot shut down one thing without knowing what it does. Absolutely, the mouth is not supposed to breathe. But if they have a compensated airway, and that's why the real term is airway, even the mouth is not meant to breathe, but we use it. Exercise and everything, we use the mouth at times. So when you're not able to breathe through the nose, your body tends to open your mouth to breathe during the day too. But in the nighttime, when that exactly same thing happens, the tongue, which is the biggest muscle, right, now is supposed to rest, have a parking spot, but is on a slope. So it goes back because the airway, because your jaws have not grown, your muscles are constricting it. So now the tongue goes rolled back and affects the whole of the structure there. That's where the first snoring sounds start coming in, which is the biggest cultural joke. Oh, he snores more than me when you talk to husband and wife. That's the most common conversation that happens in my chair. But understand if you're snoring, it is means you're already using something that was not meant to be used those eight hours. Eight hours when you're supposed to rest, you're supposed to rest. The body is in a parasympathetic tone. Gotta respect that. But instead of that, you're not able to breathe. Somebody is gently choking onto you, compromising that airway ability, and you're going in those cycles, as we call this, hypopnea and apnea. Apne is complete absence of breath, and hypopnea reduce amount.
SPEAKER_01 6:56
People, I've read, you tell me, um, that people literally stop breathing multiple times during the night. Yes, they do. I have recorded my wife doing that.
SPEAKER_00 7:04
I've recorded her snoring. My 13-year-old daughter is diagnosed with sleep apnea because of the symptoms I saw in her. I mean, it is so personal for me to know this because I was able to heal myself. And the symptoms, as the story continues, when the body is not able to breathe through the nose, uses the mouth, and the mouth is being compromised because a lot of people don't sleep in the best way possible. They are tugging the chin in in a fetal position or sleeping on your stomach where you cannot open the mouth. The mouth cannot. So, what does it do? It's there's a knee-jerk reaction. You snapped from a parasympathetic mode to a sympathetic. Now you become a warrior fighting a war that you're not supposed to.
SPEAKER_01 7:41
You're not gonna get a big thing.
SPEAKER_00 7:42
So now the grinding starts. The body's saying, I'm gonna grind you out and open the door. So then bruxism starts. So everybody who brooks, my question to them is why are you brooksing? And that's the trail I was telling you about.
SPEAKER_01 7:57
Those marks are most dentists would just give you an appliance and say, here, that way you won't wear your teeth down. Meaning the appliances don't to wear the not to wear the teeth down, but not to fix the problem.
SPEAKER_00 8:08
That is what I was almost 20 years ago. You were doing that, yeah. That's what we were taught.
SPEAKER_01 8:14
Yeah.
SPEAKER_00 8:14
The question is not what you're taught. Being a clinician, as you have realized, it's a responsibility for us to go and help ourselves. As I tell all my patients, suffering is given to me so that I can heal myself and heal millions. And that's how it should be.
SPEAKER_01 8:30
Yeah, absolutely. Uh, were you interviewed in the biodentistry documentary? Yes, sir. Yeah, right. So we're in Cannes, where these documentaries get featured. And uh, did you talk all about this?
SPEAKER_00 8:40
Yeah, yes, I did open that doorway, a segue, but this is a body of knowledge that needs to be understood, and people need to ask this question. So, yes, I spoke about the sleep apnea, the grinding part. Because when I take this story a little further, what happens is when you're grinding, the mechanism of grinding is definitely happening from your brain because the brain loves to have an equilibrium, a set point. So, whenever we're trying to balance ourselves, our teeth clench. So that's an anchor point. Now, because of this grinding, when the teeth are worn down and your joint here, which is on both sides, is suffering like a knee joint where it's losing the cartilage. So you have a compromised bite. So instead of me holding on your hand firmly, it's a compromised bite. Hold. When the compromise sets in, all the muscle fascia tension builds up. And the first gear is right from your first cervical vertebrae down to every first symptom that a person experiences in terms of plantar fasciitis, knee pain, hip pain. Every single joint in the body is absorbing that stress because you're grinding and clenching.
SPEAKER_01 9:42
So that can affect all those joints you just mentioned, right?
SPEAKER_00 9:45
This is this is biomechanics. Biomechanics and understanding that nothing is isolated.
Grinding, TMJ Stress, Whole Body Pain
SPEAKER_00 9:52
The mouth is connected, the mind is connected, and your intentions are connected.
SPEAKER_01 9:56
Yeah, that's amazing. Yeah. So that's the way you look at things now. And I I think your background had a lot to do with that, right? Is that understanding? I I want you we didn't talk about the solutions for the airway.
How To Spot And Diagnose Airway
SPEAKER_01 10:08
You mentioned a device, and sometimes that can fix it, but talk about all the solutions, how we correct airway, and then also how do you um how do people get checked for airway?
SPEAKER_00 10:17
So let's start with checking out first, and then also the solution. The checking is that if your spouse, your friend, anybody who's snoring, that's a wake-up call. Yeah, most likely they have an airway issue. If you're waking up in the morning and you feel like you need to sleep a little bit more, you feel headaches. Most likely you have an airway. Yeah. You have aches and pains in the body. You have the desire to spend a ton of money going to massage therapist, chiropractor, not abusing in any way, but you feel that need, it means your body is storing the stress. I always say the body is a hard drive, it stores both the good and the bad. Okay. And if it's hurting, it's selling your signal that you need to be checked. So the damage from grinding is stored in the body. Yeah. The damage from not getting enough air is compromised to all the organs which are trying to be on a circadian rhythm for eight hours and trying to refuel, rest, regenerate, but they're not getting it. Now, all these organs are running at a compromise state. So they're going to chronic distress, which causes chronic illnesses in people. So all people who have minor symptoms to major symptoms, you have to know how you breathe. Are you breathing it right or not? So it's a question that can be openly discussed with your dentist. I have grinding, I clench.
SPEAKER_01 11:32
Or I my partner says I snore. I mean, assessment of the you know symptoms someone has. Yes, check one. What's two?
SPEAKER_00 11:40
So it's the number two is that when they have caught themselves clenching during the day, which is the most common thing. They will say, Yes, I did it. I but I'm stressed. I'm stressed. And third is when they are going through all these symptoms, when they're rest in the night, they are not getting the restful sleep they have been craving for. And once in a while that sleep happens. That's of those are some of the wake-up cult. You have pain in your joints, you have grinding patterns, and a lot of times a dental work is gonna fail. And the dentist would know better that these are my TMD, uh sleep apnea, and grinding patients. And they need to change the lens and look at things. And so do the patients need to understand that if your dental work is failing or you have these symptoms, yeah, because most people don't know they grind.
SPEAKER_01 12:31
And they'll say, No, I don't grind.
SPEAKER_00 12:32
This is but they it's it's it's an you know, we do something which we feel doesn't harm us, but a specialist can look at things and say that this is the trail. So let's walk together and see where it leads to.
SPEAKER_01 12:46
Now, what about look assessing it with a cone beam? Actually, a cone beam shot at a certain angle to actually look at the airway itself.
SPEAKER_00 12:52
Great question. So certain cone beams have a FOE, as they say, that area of focus where you can span the whole airway.
SPEAKER_01 13:00
Yeah.
SPEAKER_00 13:00
You check the airway with a scan and then you check it with an appliance. Now, appliance is one of the possibilities, even though I mentioned a couple of times. You check it, you see almost 50 to 80% improvement in the amount of space that can be expanded.
SPEAKER_01 13:16
Yeah, so does that tell us?
SPEAKER_00 13:17
With the cone beam, we can actually look at the space itself. See the whole flow of the amount. So you can see the uh the laminar flow of the air when it's going through, the borders are defined. And when it's constricted, you'll see the colors of red, yeah, you know, orange, and then the green.
SPEAKER_01 13:32
And then you're saying you can put an appliance and see what correction it makes.
SPEAKER_00 13:36
So one of the things, that's one of the things there's a treatment that can be done by laser modification for soft tissue
Treatments Beyond A Night Guard
SPEAKER_00 13:42
improvement. There's a treatment, you do one more. Now we're kind of moving into some solutions, right? Absolutely. So the laser night lays. Night lays by Photon. They're one of the brands. And what laser does, it gives the soft tissue which is sitting on the bony part. So that's quick relief. So you do a couple of sessions with that, and that will start opening their airway in a way which will take that pressure and the stiffness those muscles have gone through because of the chronic snoring and the inability to breathe properly. So it starts there, but that's something I can do really fast, or any other dentist can do it who's trained in the laser technology. But then once you have the understanding, you do a proper setup of their jaw bite and understand. Is it an orthognatic problem? Is it an orthopedic problem? Don't just move the teeth. Right. When your intention is just to have perfect white teeth and a compromised airway, what good have you done to that individual? Exactly. That's my question. And that's why straight teeth are not the answer. Open airway is an answer for health.
SPEAKER_01 14:42
Yeah, exactly. That's right, open airway. But again, um, I have seen you mentioned a little bit of like the soft tissue, meaning the tissue around the hard bone, right?
SPEAKER_00 14:51
Yeah.
Hidden Jaw Infections And Cavitations
SPEAKER_01 14:52
Um, your tongue being part of that. But I have seen people with a lot of infection in their mouth have so much inflammation that once they clear the infection, a lot of their breathing problems goes away. So we know infection can be a cause of this as well. Absolutely.
SPEAKER_00 15:07
Now we're going to more gross from the subtle to the more solid part of it. So the breath work, that which I love and see because I've been into the meditation and understanding breath. So that gives me a self-authority to that and a self-responsibility that I need to help people with that. But now when I go to the dental side where I love to do surgeries for people to help out with the infections in the bone, so an infection in the bone is gonna sit there for decades, untreated, undiagnosed, and you have these blowups that are happening in the rest of the body because there can be bacteria, fungus, mold, parasites all sitting there, and that's gonna clean when the infections are sitting there. Root canal-treated teeth or work, that was not done in properly.
SPEAKER_01 15:48
Root canals can be these infections you're talking about, where teeth were removed, like wisdom teeth removed, they can pull the tooth out, it heals over, no pain, but there can be infection behind there, we call it a cavitation.
SPEAKER_00 16:00
They're hiding there under the facade because the gums have closed. But we all know there are anaerobic bacteria that can survive. And there's so many species that can survive in the depth of the ocean, so they can survive in the bones. So they are growing there, thriving there, and they're sending signals where they're blocking the flow of the energy in your body, and that needs to be recognized. Yeah, that needs to be cleaned up. As soon as you do that, there are so many testimonials of miraculous healing that happened.
SPEAKER_01 16:26
Yeah. Talk about some of the things you've seen, just fixing an airway, you know, and I'm sure there's others for
Real World Changes After More Oxygen
SPEAKER_01 16:34
infection.
SPEAKER_00 16:34
So I had a 75-year-old uh woman come to me, sent by a chiropractor, had a hidden hidden infection in the jaw, we cleaned it up. She never knew she had an airway problem. So I cleaned that infection, but the airway, I saw the signs, she gets a sleep study done, comes out positive. Sleep study is read by an MD sleep specialist. Now, this is where we start liaisoning with the medical community.
SPEAKER_01 17:01
And by the way, the sleep study is another way to diagnose this problem. Sleep study is a right way. Cone beam is one way, C-O-N-E, B-E-A-M, cone beam, or a sleep study.
SPEAKER_00 17:09
Yeah, C T scan is a common word people understand. Cone beam is a dental word because where we have cone beams that are going on, they have walk cells instead of big cells. But that's a diagnostic tool. You're absolutely correct about it. So you take that CT, you do that. So I diagnosed her with the help of a medical doctor who gave this uh scan uh for the uh an appliance that was used, and he interprets a sleep study. We get the results, and what are the three things that come? It's all in the lap of a dentist, orthognatic, orthopedic treatment, laser treatment by an ENT or by Pertona, as we said, and third is the last one is an appliance. But by the time I have done the orthopedia, orthognatic and the laser, there's hardly a need for an appliance.
SPEAKER_01 17:52
Yeah.
SPEAKER_00 17:52
If it is done correctly. That's the power of an airway dentist.
SPEAKER_01 17:55
Yeah.
SPEAKER_00 17:55
Because now you open up the channel, so everything can flow back and forth and forth. And without oxygen, what are we?
SPEAKER_01 18:00
So, anyways, you didn't finish the story though. Um without oxygen, yeah. Without oxygen, it brings disease and then death, early death. Um, okay, so the the story, and that uh you were telling a story about the person with the airway issue. Right.
SPEAKER_00 18:14
So once she was diagnosed with the airway, she comes back to me and she tells me, Doc, I used to take naps at two o'clock every single day. I don't do that anymore.
SPEAKER_01 18:26
Yeah.
SPEAKER_00 18:27
Subtle changes. People's brain fog, their ability to think clearly in stress, their ability to deal with emotional challenges in life, changes. Every time we we hear a stressful situation, what do we tell? Take a deep breath. I mean, how difficult it is to understand that the breath is your segue for every cell in the body to be perfused correctly and give them that power that they create the energy you need and detox every cell. So we're talking cellular. I'm talking my language. Absolutely, and that's why our works align together. Yeah, we do the same work we love to do.
SPEAKER_01 19:08
So I'm the number one referral to biological dentists for airway, for all of these things, infection, um, because so much disease starts here. Okay,
Titanium Implants And Galvanic Currents
SPEAKER_01 19:20
you know, you spent a lot of time in Germany studying under one of the best in the world, Gahani himself. Speaker Ganati is Gannati himself. And um uh one thing that you learned a lot about is titanium implants. Many people watching this have titanium implants, and unknowing to them, it can be problematic. Talk about it.
SPEAKER_00 19:39
So the titanium implants over the period of time have been doped with cheap metals in them, and the body has degraded. So now you're getting cheaper implants. But what are you getting? You're getting a post that have an ability to conduct galvanic currents with the old mercury fillings.
SPEAKER_01 19:58
Galvanic currents are an electrical current, it's a battery effect in the mouth right next to the brain. We are nothing but energies.
SPEAKER_00 20:05
And if you have metals with concentrated amounts in certain areas, that is definitely gonna impact your body's ability to transfer that energy in different parts and in different tissues. So when you have titanium implants and you have that inflammation around the gums, which they call it implantitis, like the gum has gingivitis, implantitis, and it keeps going on, it's not gonna heal. You need something that is friendly with the gums. That's why at this point of time, zirconium implants have a unibody, a uni substance ability to accept the bone and the soft tissue in a way that it doesn't interfere with the rest of the Yeah, you answer the next question because people are saying, What should I have?
SPEAKER_01 20:49
And it's a zirconium type of ceramic, very strong.
SPEAKER_00 20:54
Yes, it is, absolutely. And I'm working with my uh PhD and master's thesis in Brazil with professors there, where we have found studies, and I have some papers published too, that there are inflammatory markers in patients with titanium implants that are closely affecting their ability to have the fullest capacity of their immune system.
SPEAKER_01 21:17
I've seen it all the time when people get their titanium and they they have no idea it's affecting their health or their immune system. They get the titanium post out, you know, meaning the uh the implant, and their life changes. Their autoimmune backs down, their immune system changes.
SPEAKER_00 21:33
It's like a trophic light. It just goes green and they start healing so fast and rapidly that 90% of the time people just tear down when they come back and say, Oh, they healed. And that's the best feeling I have in the world. When people say, Should I get it out?
SPEAKER_01 21:47
I'm like, absolutely. I mean, this is me. I'm like, you know, if you care about your health, get it out, right? Get a get a uh ceramic implant, a zirconium implant. When you know better, why would you go back? That's right. Exactly. I mean, and that's the purpose of doing shows like this, is just so you know, people know better. How does, you know, with your background, how does this all affect?
Oral Gut Axis And How To Find Him
SPEAKER_01 22:08
I mean, you kind of led to it, you know, but the mind-body spirit, right? I mean, it affects everything of who we are as a human being. And I I people are gonna think that I'm being over-exaggerative about how this, the oral, you know, cavity affects. But listen, it's not my estimation that it's estimated to create cause 84% of chronic illness. So it's physical, spiritual, and emotional.
SPEAKER_00 22:32
How? Oh, you just died it correctly, spiritual, emotional. The body has different metals, as we were discussing. And what's the wiring? The wiring is a saliva. Saliva is the electrolyte. When you have distant water, it cannot conduct electricity. When you put ions in it, it starts conducting electricity. Saliva is filled with so many different molecules, ions, and that has the ability to conduct that galvanic current.
SPEAKER_01 22:57
Yes.
SPEAKER_00 22:58
Now that saliva flows down into the gut, and you have the gut and oral axis that starts. And that axis, everything that is in the gut and in the mouth is going and influencing over there. So when you're saying, how can it not move from the mouth? There's already a path. There are segues going from the nervous system to the brain, there are segues going to the gut, there's segues going in your in your in your endocrine organs. Every single organ they're communicating, nothing is isolation. So when they're coming and they're talking and they're not getting the help they need, plus they're loaded with the currents that are not meant to be there, there's going to be an impaired healing that's happening there.
SPEAKER_01 23:38
Absolutely. And that's a job to be done. That's it. How do people find you? Because people want to find you right now, because they're they're suspicious.
SPEAKER_00 23:46
I'm here in Stafford, Virginia, a small place, 25 minutes from DC. Not far. Uh Corande Biological Dentistry, as I say, where healing begins.
SPEAKER_01 23:57
Carrande Biological Dentistry. That's is that your website? That's my website.
SPEAKER_00 24:03
Spell it. Biological dentistry, where healing begins.
SPEAKER_01 24:12
There you go. Get assessed by this guy for airway, man. Thanks for being here. All the way, Cannes France. All right. All right. Stay tuned for the next episode, right here from Cannes France, Dr. Pompa podcast. Thank you, Dr. Pompa.

Reimagining Dentistry Through Airway Science, Biology, and Regenerative Healing
Dr. Sachin Karande is a Columbia University–trained dental physician and founder of Karande Biological Dentistry in Stafford, Virginia, where he is helping redefine the future of dentistry through a systems-based approach that integrates airway science, regenerative biology, and whole-body physiology.
In Dr. Karande’s view, dentistry is entering a profound transition.
For more than a century, the profession has largely focused on repairing damage. The next era will focus on understanding why disease develops in the first place—and how restoring structure, breathing, and biological balance can support the body’s capacity to heal.
His work explores the intersection of craniofacial structure, breathing, inflammation biology, and systemic health.
Dr. Karande earned his Doctor of Dental Surgery from Columbia University College of Dental Medicine, where his training emphasized systemic pathology, inflammatory mechanisms, and the oral–systemic connection. Columbia’s medically rigorous curriculum cultivated a diagnostic philosophy grounded in physiology rather than procedure-based dentistry.
He later pursued advanced education at Tufts University School of Dental Medicine, completing training in Integrative Biological Dental Medicine, Oral and Maxillofacial Pain, and Temporomandibular Disorders (TMD). This work reinforced a clinical model that aligns structural correction with immune regulation, regenerative healing, and long-term physiologic stability.
